Registered Nurse (RN) Needed in Sylmar, CA

Adult Day Health Care (ADHC) is a community-based day health program that provides services to older adults and adults with chronic medical, cognitive, or behavioral health conditions and/or disabilities that make them at risk of needing institutional care. We are looking to hire an eager, energetic, and kind Registered Nurse (RN) to assist our clients.

Qualifications:

- Holds a current license from the California Board of Registered Nurses.

- Is 18 years of age or older.

- The RN will provide the center with a health examination, signed by a physician, and a TB clearance, which is not more than 12 months prior to employment or within seven days of employment.

The duties of the RN include but not limited to:

- Providing observation and monitoring of the participant’s health status.

- Providing nursing care within the scope of practice for participants as specified in the individual plan of care.

- Providing health education and counseling to participants and their families, when indicated, in their plans of care.

- Providing personal care services for the participants as required by the individual plan of care.

- Providing continence training programs as indicated in the plans of care.

- Providing emergency care, including transportation arrangements when necessary.

- May serve as a member of the Utilization Review Committee.

- Documentation of daily participant monitoring, and any unusual findings.
