Home Health Intake Coordinator Needed in North Hollywood, CA

Job description
Looking for an experienced home health Intake/ Staffing/ Posting coordinator who works well with a heavy workload.
Job duties include:
- verifying patient eligibility
- coordinating with hospital, SNFs, rehabs, and MD office to accept/decline referrals
- creating patient intakes
- staffing intakes to field RN's
- following up with RN to make sure the patient has been assessed
- staffing proper disciplines to follow-up with patient car
-following up with disciplines and posting visiting notes
Job Type: Full-time
Pay: Staring from $20.00 per hour
Schedule:
8 hour shift
Monday to Friday 9 a.m.- 5 p.m.
