Social Worker Needed for Adult Day Care Center in Burbank, CA

-Conduct psychosocial assessments for new participants and develop individualized care plans.
-Provide ongoing support, counseling, and crisis intervention to participants and their families or caregivers.
-Serve as a liaison between the participant, family, and care team, ensuring communication and coordination of services.
-Assist participants and families in accessing community resources such as transportation, home care, housing, financial assistance, and mental health services.
-Advocate for participants’ rights and well-being in accordance with ethical and legal standards.
